Summary ObjectiveCompassCare is currently seeking a talented and skilled full-time Clinical Services Manager to join our Clinical Team. This individual is responsible for identifying and coordinating needed services as dictated by the medical treatment plan, nursing assessment and the client environment, with the end result being exceptional care for each client. This position requires travel within CompassCare territories to ensure the overall implementation, delivery, and coordination of Client Care Services. This full-time, salaried position offers full benefits and equity interest. Primary Responsibilities: Perform initial and ongoing client assessments, identifying individual client needs in order to provide high-quality care.Develop and maintain client care plans, working in partnership with Physicians, Physical Therapists, Occupational Therapists and other professionals as needed.Provide medical care for clients as needed, including medication management.Instruct and train staff regarding each client's needs and care plan. Ensure that each care plan is being followed and fully documented. Maintain, develop, and update in-house clinical education and training materials. Other duties and responsibilities as assigned. Desired Skills: Ability to work with a variety of health care professionals and clients.Strong interpersonal skills for developing relationships with clients and caregivers.Knowledge of Advance DirectivesTechnical skillsTeam playerAbility to multi-task Qualifications: Bachelor's degree in social sciences, social work, nursing, or a related fieldRegistered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice as required by New York and Connecticut state regulations.2+ years of experience as an RN or an LPNExperience in direct patient care and assessments/observation of clinical conditions; must have strong clinical skills.Knowledge of age-related illnesses, including understanding of Palliative & Hospice Care.Excellent verbal, written, and communication skillsCPR and First Aid CertificationsStrong attention to detailMust be able to work flexible hours, including evening and weekend hours. Overnight availability may be necessary at times.Ability to work onsite in the corporate office in Stamford, CT, as well as ability to travel throughout CompassCare's area of service.COVID vaccinated. Preferred: 5+ years of experience as a Clinical Manager, Coordinator of Care or RN Supervisor within a certified homecare or home healthcare company.Experience working with older adults.
