ALCOHOL LIABILITY AGREEMENT Please read the following agreement and when purchasing ticket(s) indicate "yes" or "no" to the Alcohol Liability question. I, as the responsible party, assume all liability for myself for any form of negligence that may arise from my consumption of alcohol at the PHS Class of 1986 35th Reunion. I hereby agree to indemnify, defend, release, and hold harmless the Event Contract Signee & Reunion Committee, from all liability to me, my invitees/guests, my next of kin, my conservators, assigns, heirs, guardians or other legal representatives for any and all claims, demands, losses or damages, suits, fines, including court costs and attorneys’ fees, for any injury, death, or damaged property arising out of my consumption of alcohol at this event. I hereby waive all legal rights to pursue any form of legal action against the aforementioned parties.
