We are inviting the CU community to join us for an evening of food and fellowship, including a 9-hole round of golf. Sigma Xi Chapter of Omega Psi Phi Fraternity, Inc. - Notice of Athletic Event Participation Waiver By registering and participating in this event, you hereby acknowledge that you are a registered participant in an activity sponsored by the Sigma Xi Chapter of Omega Psi Phi Fraternity, Inc. You understand and agree that you are participating in this event on your own free will and accord and that Sigma Xi Chapter, nor Omega Psi Phi Fraternity, Inc., nor its insurer(s) will share in or accept responsibility for any liability for bodily injury, property damage, medical expense or other loss that may arise from my participation in this event. You further understand and agree, and have no expectation that Sigma Xi Chapter or Omega Psi Phi Fraternity, Inc. will provide any form of security or other measures of safeguarding for this event, as there is no reasonable expectation that such will be necessary. You further understand and agree that this event is considered a “no-fault” event by you, as well as Sigma Xi Chapter, and Omega Psi Phi Fraternity, Inc. and in the event of bodily injury, property damage, the necessity of medical expenses, or other loss, you agree to incur your own expenses without input or participation from Chapter, or Omega Psi Phi Fraternity, Inc., or its insurer(s).
