Stay tuned for more details on our meet and greet being held Friday evening, October 15th Saturday, October 16th Registration starts at 7 am. The Annual Meeting starts promptly at 8 am and will conclude at 4 pm. Please note that all events take place on the 2nd floor of the Crown Plaza Hotel. FMDSA reserves the right to capture and broadcast digital media content in the form of photographs, live-streaming, audio, and video during the FMDSA Annual Meeting 2021. By registering for the FMDSA Annual Meeting, you grant FMDSA and its representatives permission to photograph and/or record you during the in-person events and distribute, broadcast, use, disseminate such media without further approval. FAQs Member Early Discounted Registration. An FMDSA 2021 registrant must be an active member at the time of registration to qualify for the discounted member registration rates. Hotel Reservation. The Crowne Plaza Cleveland at Playhouse Square is located at 1260 Euclid Ave in Cleveland, Ohio. Reservations should be made by calling the hotel directly at 216- 615-7500. Mention FMDSA to receive our discounted rate. Potential cancellation due to COVID-19. The health and safety of our participants is our number one priority. Therefore FMDSA continues to closely monitor the COVID- 19 situation and evaluate the possible need to postpone or cancel the conference. We will work with our team of medical professionals and follow the State of Ohio guidelines to continuously evaluate the circumstances. If any changes are to be made you will be notified immediately. In the event the conference is canceled a full refund will be issued. COVID-19: There is an inherent risk of exposure to COVID-19 in any public place where people are present. COVID-19 is a highly contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and individuals with certain underlying medical conditions are especially vulnerable. By attending any portion of the 2021 FMDSA Annual Meeting in person, you voluntarily assume all risks related to exposure to COVID-19. By choosing to participate in person, you confirm that you: Are not experiencing any of the following symptoms of COVID-19 identified by the Centers for Disease Control and Prevention at CDC.gov, including but not limited to: • Fever or Chills • Cough • Shortness of breath • Difficulty breathing • Fatigue • Muscle or Body Aches • Headache • New Loss of Taste or Smell • Sore Throat • Congestion or Runny Nose • Nausea or Vomiting or Diarrhea And that you have not been in contact with someone with confirmed or suspected COVID-19 symptoms without completing a 14-day quarantine. What are my transportation/parking options for getting to and from the event? Getting downtown from the airport takes 25 minutes via a cab, Uber or Lyft. There is also a train located at the airport that takes you to the central downtown area. There is valet parking provided for a fee at the hotel. You may find less expensive parking options at garages located within walking distance of the hotel. Cancellation Policy. Cancellations received in writing on or before October 1st will be refunded minus a $25 administration fee. No refunds will be made after October 1st. The Fibromuscular Dysplasia Society of America, Inc. was founded on March 11, 2003 and received tax exempt status on October 7, 2003. FMDSA is a Delaware nonprofit corporation which is classified by the IRS as exempt under IRC Section 501(c)(3) and as a "public charity" under IRC Section 509. Donations from individuals and corporations are tax deductible to the extent permitted by law. FMDSA has become the recognized leader in the support of Fibromuscular Dysplasia (FMD) awareness, education and research. We have achieved our goal by successfully raising money for the purpose of awarding FMD research grants, building awareness programs, and educating the public and medical communities about FMD. We are a not-for-profit organization and programs like these can not succeed without your support. Through volunteerism, membership, financial contributions, and event participation, the FMDSA can fulfill our vision and improve the quality of life of those affected by Fibromuscular Dysplasia.
