Looking for participants who are medicaid enrollees, diagnosed with Type II Diabetes.
We will help you adopt healthy eating habits while covering your grocery expenses for the duration of a 3 month program. Additionally, upon successful completion of the program, you will receive $180.
As a part of the program you will receive:
- Fully covered groceries (for you and your household)
- $180 in addition to groceries
- 1:1 health coaching
- Personalized recipes
