Are you a diagnosed with Type II diabetes and currently enrolled in Medicaid?
If your answer to the above question is yes, we will help you adopt healthier eating habits while covering your grocery expenses for the duration of a 3 month program.
As a part of the program you will be provided with weekly grocery deliveries (for you and your family members), personalized health coaching and curated recipes.
Additionally, upon successful completion of the program a payment of $180 is granted.
