Description
Looking for volunteers who are medicaid enrollees, diagnosed with Type II Diabetes.
You will be provided with fully covered groceries and recipes to help you adopt healthier eating habits.
If you are eligible you will receive the following:
1) $180 payment, upon successful completion of the program (in addition to fully covered groceries)
2) Fully covered groceries for you and your immediate family members
3) Tailored recipes and coaching by certified health coaches
