Living Well with Diabetes is a peer-led, community-based intervention that helps individuals with diabetes or pre-diabetes learn how to manage and improve their health.
During the 6 two and one half hour sessions, participants will learn:
Nutrition/healthy eating and preventing low blood sugar
Glucose monitoring
Appropriate exercise and maintaining a balance of blood sugar
Communicating effectively with family, friends, and healthcare providers
Techniques to deal with pain, fatigue, frustration, and isolationMaking informed treatment decisions and disease-related problem solving
Medication usage
Skin and foot care
