Free sports physicals will be provided to any middle school student athlete participating in sports within the Independence School District.
Athletes must bring all 3 MSHSAA Pre-Participation Physical forms found on the link below and should complete pages 2 -3 prior to arrival. There will be an additional consent form that you will receive when you check in on the day of the event which must be completed and signed by a parent or guardian. Any student athlete without the proper paperwork will be turned away.
Walk-ins will not be accepted at this event.
