EVERETT- Middlesex District Attorney Marian Ryan and Everett Chief of Police Steven A. Mazzie have confirmed an investigation into a fatal industrial incident that occurred today at a storage facility located on Commercial Street in Everett.
Preliminary investigation suggests that a 38-year-old male was working alone on a forklift hydraulic system when the machine collapsed on him. The male was pronounced dead at the scene.
This is an open and ongoing investigation being conducted by the Middlesex District Attorney's Office, the State Police Detectives assigned to that office and Everett Police. The Occupational Safety and Health Administration is also investigating.
