By accepting to attend church service, you agree to the following Covid guidelines:
You answer “no” to the following questions
1. A body temperature over 30° or a sense of having a fever.
2. You have chills, cough, sore throat or trouble swallowing, shortness of breath or difficulty speaking, muscle aches, headache, decrease of loss, smell, or taste.
3. You do not experience nausea, vomiting, diarrhea, or abdominal pain.
4. You have a runny nose, stuffy nose or nasal congestion, not feeling well or experiencing extreme tiredness.
5. In the past 14 days have you tested positive for COVID-19?
6. Are you waiting for the results of a diagnostic COVID testing?
7. In the past 14 days have you been in close contact with anyone with presume I’ll confirm COVID-19?
If you answer yes to any of the above you will not be permitted to enter the church, we apologize for any convenience this may have caused. Thank you for your cooperation
