Vocational Coach (ID/DD population) Freeport – Full Time (35 hrs/wk, flex) $16/hr. AHRC Nassau provides support to over 2,200 people throughout Nassau County. Our organization partners with a family of organizations including Brookville Center for Children’s Services, Citizens Options Unlimited and Advantage Care Health Centers. We have a unique opportunity for a Vocational Coach to work with AHRC’s Supported Employment program which provides job development, placement services and ongoing support for people with Intellectual and Developmental Disabilities to maintain employment. In this rewarding role, you will: • Provide support to the people we serve in community and facility based vocational activities • Support the development and ongoing revisions to individual’s Staff Action Service Plan by providing input and information • Track individual’s progress towards desired goals and objectives in Life Plan • Segment job duties and assignments into specific steps to facilitate the learning of the job • Monitors individual’s level of performance including determining when support can be maintained, increased, or gradually faded • Monitor the quality of the work that the people we serve produce • Serve as a liaison between people we serve and worksite employees/employers to facilitate appropriate work readiness skills training and professional work relationships • Maintain accurate records on job coaching services provided • Document and recommends to supervisor any action necessary to overcome employment barriers • Respond to crisis situations in a humane and proactive manner and in accordance to policy We require: • High School diploma or GED required • Six (6) months of employment experience (Preferred) • Good math and communication skills • New York State qualified driver.
