Found COVID-19 Vaccine Card on 7/15 belonging to an individual with initials M.S.H., DOB Year = 1999. Laminated card. Found near corner of NW Tyler Ave. and 14th or 13th St. Please email me with your full name and Date of Birth if you believe this is your card and I'd be happy to return it to you. Good on you getting vaccinated! I just didn't want it to blow into the street and into a storm drain so I decided to pick it up b/c it's an important health record.
