Job title: Specialized Service Worker
Work on site with clients residing in nursing facilities. Locations of nursing facilities include Framingham, Milford, Northbridge vicinity. Provide support to individuals with disabilities to maintain skills that will foster personal growth and independence (Active Treatment).
Responsibilities include assessing needs and developing care plans, assisting clients carry out therapeutic goals, providing opportunities for community reintegration, documenting progress and collaborating with multidisciplinary team. Report to Active Treatment Supervisor.
Great position for someone seeking hands on social service experience! Must have your own transportation.
CTRSs, CNAs, Activity Professionals or other healthcare providers encouraged to apply.
