Pre-Season Sports Physicals for Middle and High School Students Please read through all of the information below detailing how we are handling our physicals this year due to the continued COVID-19 pandemic. Physicals are free of charge. Located at Mercy Personal Physicians at Lutherville. We will not be accepting walk-ins and are limiting the number of athletes allowed per half hour. Only registered athletes will receive a physical. Athletes must come at their designated time only. You must register for a specific time slot from the drop down menu during the checkout process. Masks must be worn properly (over mouth and nose) and are required at all times. Athletes may bring one parent or guardian with them only. No other visitors will be permitted inside during the physical due to social distancing protocols. Do NOT enter the building until you are instructed by staff. Social distancing guidelines will apply. No line will be allowed to form in the lobby. Please be prepared to wait outside if there is a wait. Athletes and parent/guarding must pass COVID screening questions upon arrival. Please STAY HOME if you are not feeling well. Please be sure to arrive on time for your appointment time. There is no guarantee that we will be able to perform the physical if you are late. Physicals are for middle or high school students at either public or private school. We will not take elementary school students or college students. There is free parking available right in front of the building. Below is a link to the form to print and fill out before you arrive. Pre-Participation Physical Evaluation Form If you do not bring your own form we will have the standard Maryland Public Secondary Schools form available at check in. If you have a different form specific to your school, you are welcome to bring it with you. The physicals typically take about 30 minutes, this is not including the additional screening and precautions mentioned above. Your student will see an athletic trainer or physical therapist, then an orthopedic physician, then a primary care physician, nurse practitioner, or physician's assistant for a fully comprehensive sports physical. Athletes are responsible for submitting their completed physicals form to their schools. ATI and Mercy are not responsible for lost physical forms.
