On behalf of myself and/or my child/ward (as the case may be) and as a condition of myself, and/or my child/ward attending summer camp this year, that I hereby acknowledge and agree to assume all COVID-19 and similar risks, known and unknown, associated with attending this year’s summer camp, EVEN IF ARISING FROM THE NEGLIGENCE OF the organizers and the host facility including its board members, employees, officers, agents, volunteers, insurers or representatives (hereinafter collectively “RELEASEES”) and I ALSO UNDERSTAND AND AGREE THAT organizers and the host facility does not assume any responsibility or obligation to provide financial or other assistance, including but not limited to medical, health, or disability assistance, including but not limited to medical, health, or disability assistance in the event of injury or illness, and I hereby agree to RELEASE, INDEMNIFY AND HOLD HARMLESS all RELEASEES from and against any and all such claims, actions, suits, damages, liabilities (and the like) arising out of or in connection with, directly or indirectly, COVID-19 and the like, including, but not limited to, costs, expenses and attorney fees.
