Press release from Waukesha County: June 5, 2021 The Aging and Disability Resource Center (ADRC) of Waukesha County is administering the Senior Farmers' Market Voucher Nutrition Program (SFMNP). Seniors who are at least 60 years old (Native Americans 55 years or older) and have household incomes 185% or less than the federal poverty income guidelines are eligible to receive $25 worth of vouchers to purchase fresh, nutritious, unprepared fruits, vegetables, and herbs from participating farmers' markets and roadside stands. Vouchers are distributed on a first-come, first-served basis and are limited to $25 per household. Vouchers will only be distributed via mail and at five scheduled community distributions. The ADRC will be accepting completed eligibility agreements directly from clients and/or their proxies via US mail or fax. Once the completed agreement is received, the ADRC will mail the farmers' market vouchers directly to clients. For community distribution events, the participant or authorized representative must visit the distribution site in person to receive the vouchers. A proxy can represent up to ten individuals and must provide a signed statement from the eligible senior designating the individual as his/her authorized representative. The proxy must also have the full name, address, phone number and date of birth of the eligible senior to complete the application. The ADRC will distribute the Farmer's Market Nutrition Program vouchers throughout the county during the events list below.
