This meeting will be in-person with a limit of 10 people attending and unlimited availability virtually. Simple things they can do to cope with the stressors and losses of caregiving; v How they can care for themselves so they can care for their loved one; v Resources available to them; v Information that will simplify the caregiving process; v How to have difficult conversations; v To talk about end-of-life care and make important decisions; v To maintain a positive, healthy and hope-filled outlook.Sara Chambers, BSN, RN, CHPN is the Community Educator and a Certified Advance Care Planning Facilitator for Hospice of Northwest Ohio. With twelve years of nursing experience, mostly in critical care and hospice, she feels honored to have found her niche in nursing. Sara upholds a national certification in Hospice and Palliative Care. She passionately believes in advocating for patients, and their through education related to: the Hospice philosophy, advance care planning, death and dying, and symptom management at the end of life. Some of Sara’s responsibilities at Hospice of Northwest Ohio include coordinating requests for outreach opportunities for the community and assisting with developing and/or presenting any new educational offerings. She also provides free Advance Care Planning to anyone in the community who would like assistance in completing Advance Directives.
