CCHF's first major in-person event in more than a year. By completing this registration form, we hereby certify that: • We all reside full-time in the same household in CCHF’s service area at the address shown on this form. • We understand that completing this application does not guarantee participation. If our application is approved, we will pay a $100 deposit by July 9th and that if we fail to do so, our participation will be canceled. We understand that the deposit will be returned to us upon check-in at the hotel in the form of a Great Wolf gift card than can be used to pay for incidental expenses during our stay. We also understand that if we cancel our participation at any time after July 9th, we will forfeit this deposit. • If our application is approved, we will sign a waiver of liability absolving CCHF and the hotel of any blame for all health issues that might arise from our participation. • That CCHF will pay for two night’s lodging, including taxes and the daily resort fee and will provide each participating family with one $50 resort gift card per child for to pay for each kids’ activities and one $75 resort gift card for each family member to help with meal expense—and that CCHF will also host group breakfasts on Saturday and Sunday. But all other expenses—dinner Friday night, lunch and dinner on Saturday, snacks, beverages and activities such as Build-A-Bear Workshop and so on—that exceed the value of the gift card(s) will be our responsibility, that this could easily amount to $100 or more per-person of out-of-pocket expense for us, that these additional expenses will be charged to our credit/debit card (which we will be required to present to the hotel at check-in), and that the hotel will place a $200 hold on our card for incidentals. • We will not be permitted to bring in outside food or drinks. • CCHF will not be providing transportation to the event or gas cards to defray travel costs. • We will follow all locally required health and safety precautions as directed by CCHF and the hotel, including, but not limited to, the wearing of face masks and observance of social distancing requirements. The Central California Hemophilia Foundation (CCHF) is dedicated to improving the quality of care and life for person impacted by inherited bleeding disorders through education, advocacy, and support. CCHF is a non-profit 501(c)(3) organization that assists local individuals and families with bleeding disorders pursue classes and activities that will help develop skills and knowledge to enhance their mind, body and spirit.
