As a Host Home Provider, you will have the opportunity to change someone’s life for the better. As a Host Home Provider, you will serve an intrical part in the persons’ individualized plan of care. What will your role be?  Welcoming someone into your home and family  Provide person-specific care (i.e. ADLs, medical appointments, medication administration, community integration, individualized goals, etc)  Foster independence and self-determination  Ensure proper health & safety guidelines are in place What will you receive??  Education and on-going skill development to learn how best to provide quality care  Opportunities to network with fellow mentors on best practices  Clinical supports and case coordination from MA MENTOR skilled personnel  Accessibility to 24/7 support for emergencies  Earn money for providing care  Added meaningfulness in giving selflessly We are currently in need of SINGLE female and male host homes For more information on how to make a difference and change a life please contact LeAnn at 508-368-2700 x2719
