Please join us for the 2021 Graham County Annual Community Health Improvement Workshop During this meeting, we will review activities and progress from the past year in launching Coalitions and partnerships to address the identified Community Health Improvement Plan (CHIP) Priority Issues: *Food Access and Nutrition * Mental Health * Substance Abuse* We will reassess our goals and objectives to continue moving this important work forward together. All Graham County community organizations and entities that play a role in contributing to the health and well-being of the community are invited to attend!
