Because of the hidden nature of this threat, everyone who receives approval to visit CityLight Church must rigorously follow the practices specified in these protocols. The physical and spiritual health of every person is our top priority. These protocols are designed to keep our staff and visitors safe and to help facilitate a safe environment. By reviewing and completing this form you are confirming the following: Þ You have not been in close contact with a confirmed case of Covid-19 Þ You are not experiencing a cough, shortness of breath, or sore throat. Þ You have not had a fever in the last 48 hours. Þ You have not experienced a loss of taste or smell. Þ You have not experienced vomiting or diarrhea in the last 24 hours. You have not tested positive for Covid-19 in the last 14 days. *** If you are experiencing any of the symptoms listed, please do not come to the Church location. Please stay home and contact your physician immediately. *** *Please note that we are yet to resume KidzCiti Services.* Upon entry into the church, you agree to have your temperature taken and to abide by the rules listed: Þ Have a temperature scan. Normal temperature should not exceed 100.4 degrees Fahrenheit. Þ Wear a face covering (over nose and mouth). Þ Practice recommended social distancing, which is at least six feet between people of different households, to the greatest extent possible. By registration of this event, you accept and acknowledge this disclaimer, and agree to have read and agree with all items listed.
