COVID-19 Information & RequirementsFamilies of Lab athletes who wish to attend a game or match on Lab’s campus must please do the following: Each parent/guardian spectator is required to print, sign, and bring with you this Visitor Attestation Form for yourself and a separate Minor Attestation Form for each minor family member who will be joining you. (Note: A separate form is needed for each spectator.)All visitors will be asked to wear masks and family “pods” will need to stay 6 feet away from other pods. You will be unable to access any Lab buildings while on campus.
