The beloved Church of the Good Shepherd in Vancouver, WA is ready to dawn its remodel with all the necessary COVID safety precautions! We are excited you are here to register for the 8:30 or 11:15 AM Holy Eucharist at Church of the Good Shepherd!Registration opens at 9 a.m. on Monday and closes at 7 p.m. on Saturday. There will be 40 seats available for each service. If you register and then discover you cannot attend for any reason, please cancel your reservation as soon as possible using the "View and manage your order" link in your confirmation email (Eventbrite account required). If you will be serving as a liturgical minister at this service (hospitality minster, lector, altar guild, etc), please do NOT use this form. For the health of our community, all participants at this event are required to assent to the conditions below, and agree to voluntarily assume any risk of physically participating at this event. By registering for this event, you are agreeing to all the following: 1. I agree to follow all social distancing, sanitizing, and temperature-taking protocols in place at this event or activity and agree to wear a face mask for the entirety of this event. 2. I understand my attendance and participation in the above event can pose a risk of COVID-19 or other contagious or infectious diseases to me, and if I am a high-risk person, as defined by the CDC, I should consider staying at home and avoid participation. 3. If I have been recently exposed to COVID-19, I understand that I may pose a risk to others. 4. On the day of the event, I will take my temperature and check for fever. I agree that I will stay home and not attend this event if I have experienced any of the following symptoms IN THE 48 HOURS PRECEDING THE EVENT: fever or chills, cough, shortness of breath of difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting. 5. I am not isolating or quarantining because of possible exposure to a to a person with COVID-19, or because I am worried that I may be sick with COVID-19. 6. I am not currently waiting on the results of a COVID-19 test taken because of possible COVID-19 exposure or suspected COVID-19 symptoms. 7. If there is an outbreak, I understand that my information may be shared with Public Health officials to engage in an approved contact tracing program. I understand that my information will be saved and may be released to such Public Health officials in the event it is needed for such a program.
