Pinnacle HealthCare COVID Vaccine Clinic for San Benito High School This clinic is for current San Benito High School students that are sixteen years of age or older. School faculty and parents are welcome to sign up for the clinic as well. Please right click on registration pictures below to print and fill out before event. Registration forms below are in English and Spanish and should be filled out for every person receiving vaccine. Bring a picture ID and insurance card with registration paperwork. If you do not have insurance please provide a social security number on registration paperwork. If you have family and friends that would like to to be vaccinated we will also be hosting a drive-thru vaccine clinic for San Benito County on Saturday, May 22, 2021. Time and location TBA.
