GLENVIEW, IL – After Pfizer announced earlier this week that it expects to receive federal authorization to allow teenagers up to the age of 15 to receive the coronavirus vaccine by early next week, District 225 officials said they will set up appointments for students to receive their first shot. Superintendent Brian Wegley said that the district will set up a community vaccination site for students ages 12-17 if the authorization is granted for Pfizer to begin vaccinating students in that age group. Passport Health has already updated its system to allow students to make appointments to receive a shot but Wegley that that if authorization is not granted to Pfizer by federal health officials, the appointments will be canceled. District 225 has partnered with Passport Health to offer vaccination appointments to students and their families on May 14 and May 21, district officials said this week. The vaccinations will use the Pfizer vaccine and will take place at the Glenbrook South High School cafeteria and will be offered between 9 a.m. and 5 p.m. on both days. The second dose of the vaccine would be administered on June 4 and June 11 depending on the date of the first shot. Appointments may be made by filling out this form. All students and their family members must have an appointment as walk-in vaccinations will not be offered, the district said. All students will need to have a consent form filled out by a parent or guardian and district officials said that the form should be filled out in advance of the scheduled appointment. Insurance is not required to receive the shot, but those participating in the vaccinations should have their state ID with them or a driver's license they are older than 16. Participants up to age 17 will be eligible to participate and do not need to be accompanied by a parent or guardian as long as the consent form is properly filled out. Questions about the vaccinations should be directed to Lisa Wall, executive assistant for business services by emailing [email protected].
