We are looking for victims of Wake Forest Baptist Health who's doctors/practice have miscoded annual physical exams which are covered under your medical insurance as 'annual preventive care or wellness' exams, to instead be coded as outpatient exams in order to charge the patient and insurance companies an outpatient charge. 1) Did you schedule an 'annual preventive care or wellness' exam or did Wake Forest Baptist Health contact you offering free 'annual preventive care or wellness' exam only for you to be billed for lab tests and an outpatient charge? 2) Did your medical insurance pay for 'annual preventive care' exam lab tests, i.g. Blood pressure screening Cholesterol screening Colon cancer screening — especially for people ages 50–75 Diabetes screening Hearing test Osteoporosis screening – especially for women and some men over age 65 Infectious disease screening Skin cancer screening For women only: Breast exam — Every 3 years for women ages 20–39, and every year for women 40–64 But due to the office coding it as an outpatient visit you were charged a co-pay for the exam and tests. 3) You signed a waiver form for the "Wellness/Preventive Care visit scheduled today." You knew your insurance paid 100% of this wellness exam including tests. 4) You only discussed your wellness visit and not other issues you were having knowing if you did that it would be charged as an outpatient visit as well. 5) You received billing for an outpatient visit as well as for lab test(s) as listed above. 6) Your medical insurance company only partially paid the bill and you were left with a co-payment when you believed as per your insurance policy that the wellness visit and tests listed were covered 100%. Please respond to this request. All correspondence will be kept confidential. This may become a class action lawsuit to recover your losses. I am individual who is fighting Wake Forest Baptist Health about overcharging and willfully miscoding patients visits in order to overbill the patient and their insurance companies.
