Register to attend service on Sunday, May 9, 2021 PLEASE READ ALL INFORMATION PRIOR TO REGISTERING In order to comply with CDC Guidelines please adhere to the following: *Reservations are required for each family member *No one under the age of 16 may attend services at this time *Be sure to arrive by 9:30AM at the latest to allow time for check in according to CDC guidelines *Temperatures will be checked before you are able to enter the sanctuary. If you are not feeling well, please stay at home *Please arrive with your mask properly worn (covering your nose, mouth and chin). Your mask must be properly worn during the entirety of the service *Please adhere to social distancing rules while in attendance. *Reservations are FIRST COME FIRST SERVED until capacity is reached *Please make sure you bring proof of your reservation with you. Each person must have their OWN reservation in their OWN name, therefore, the same name cannot appear on more than one ticket *Registration is subject to change due to COVID-19 and or CDC guidelines etc. We will update you on your attendance status should that happen IMPORTANT COVID-19 PROTOCOL/QUESTIONS. Are you living with or caring for an individual who is suspected or has a confirmed case of COVID-19? Have you been in contact with anyone known or suspected to have COVID-19 in the last 14 days? Have you tested positive for COVID-19 in the last 14 days? Have you traveled internationally within the last 14 days? In the past 14 days, did you have close contact (within 6 feet for at least 15 minutes) with someone with symptoms of COVID-19 or who tested positive for COVID-19? Have you attended any large gatherings (close proximity or less than 6 feet)of 25 or more people within the last 14 days?
