Job Coach – HS Diploma & reliable transportation Freeport – Part Time, meaningful, fulfilling entry-level career Mon-Fri 5p-10pm part-time 25hrs/wk Nights & Weekends Flex AHRC Nassau provides support to over 2,200 people throughout Nassau County. We have a unique opportunity for a Job Coach to work with AHRC’s Supported Employment program which provides job development, placement services and ongoing support for people with intellectual and developmental disabilities. In this role, you will: • Provide supports to the people we serve in community and facility-based employment activities • Serve as a liaison between employee & employer and facilitates worker/supervisor relationship. • Track employee’s progress towards goals and objectives • Monitor employee’s level of performance including determining when support can be faded. • Maintain accurate records on job coaching services provided • Document/recommend any action necessary to overcome employment barriers.
