HPSM members: sign up to be vaccinated on Tuesday, May 4, 2021 at the Moscone Center! Thank you for your interest in getting vaccinated and protecting yourself and those around you. Please arrive at the Moscone Center vaccination hub any time between 8:30 a.m. and 3:30 p.m. The Moscone Center vaccination hub is located at 747 Howard Street in San Francisco. To get your vaccine, please bring a photo ID and the text message you received from Health Plan of San Mateo confirming your appointment. If you do not have a photo ID, please bring your HPSM ID card or a form with your name on it. For example, a utility bill, credit card statement, or bank statement.
