Step 1. Read the COVID -19 Self Check Questionnaire on the next page at home. If you answer YES to any of the questions, contact your doctor for guidance on going to work and inform your supervisor. Step 2. Contact your doctor for guidance on going to work and inform your supervisor if ANY of the following apply to you: • Have a sick household member • Are age 65 or older Have one or more of the chronic medical conditions classified as “High Risk” for severe COVID-19 complications by the CDC and Florida Department of Health including: • Moderate to severe asthma or chronic lung disease • Heart disease • Chronic kidney disease undergoing dialysis • Diabetes • Liver disease • Autoimmune disease • Weakened immune system because of smoking, bone marrow or organ transplantation, cancer treatment, poorly controlled HIV or AIDS, or prolonged use of corticosteroids and other immune-weakening medications • Severe obesity (BMI greater than 40) Step 3. Ensure you have a clean Cloth Face Covering (CFC) to wear on the installation and in all facilities. Step 4. Make an appointment for screening at one of the checkpoints from the PEO STRI Public website https://www.peostri.army.mil/ or Share point and click on Checkpoint Appointment icon.
