Direct Support Professional - MUST HAVE FULL COVID VACCINATION Job title: Direct Support Professional Reports to: Director and Assistant Director Department: Day Program and Group Respite Location: Grand Junction Summary: This position is responsible for providing direct support, positive direction, and assistance to individuals with developmental and intellectual disabilities in accordance with program policies and procedures; and personal outcome measures. We offer paid training. Benefits: • Competitive hourly rate, based on experience (starting wage $13.00) • Flexible scheduling – including full and part time, may include some nights and weekends • Initial and on-going paid training • Paid Time Off Essential Duties and Responsibilities include the following, and other duties may be assigned: • Provides supports to people by assisting in the development and implementation of all support plans, including assisting people in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and supporting people to be independent in activities of daily living. • Maintain positive rapport with clients, providers, employees, and team members. • Model and encourage appropriate social skills in day program, in the community, and at base-site. • Implement positive behavior supports and client-specific behavior plans as necessary • Interact with individuals that may have the potential for aggressive and/or inappropriate behavior. • Maintain and expands skills and knowledge relevant to providing high quality support and services. Obtains and maintains all required certifications and trainings and applies the information to support people served. • Respect and protect client's personal information and confidentiality at all times. • Advocate for client needs and appropriate services to ensure the client has access to a high quality of life. • Read, write, complete and/or implement all areas of clients’ Individualized Service Plans. • Support the individual's rights, ability to choose, and their ability to function with or without assistance. • Ensure a safe, respectful and sanitary environment for persons in services, and fellow staff members, for example, cleaning spills at the base-site facility/community/vehicles, as needed. • Provide transportation to/from client's residence, to/from community-based activities and to/from vocational activities; communicate vehicle and transportation needs or concerns to appropriate staff. • Oversee the supplies needed for service, i.e., ensure lunch is purchased for clients who bring money; bring necessary equipment and implement activities in the community and at base-site as scheduled. • Supervise and assist clients with classes, base-site activities and community-based activities, and provide opportunities for sensory integration, learning, skill-based instruction, and/or vocational activities. • Complete and submit accurate paperwork within a 24-hour period. • Follow rules, regulations and program standards as determined by the state of Colorado's Division of Developmental and Intellectual Disabilities and Health Care Policy and Finance. • Ensure client's possessions are secured and handled responsibly. • Assist individuals with personal hygiene, eating, and follow medical protocols or procedures. • Responsibly administer and document any funds used to provide service. • Adherence to Path-Co's Attendance and Punctuality policies. Attendance and punctuality are essential functions of the position. • Effectively set boundaries with team members to promote positive decision making, judgment, and discretion to best support the clients. • Maintain a positive attitude and calm demeanor during difficult situations, modeling effective crisis management and problem-solving for others. Competencies: Customer Service - Manages difficult or emotional client situations; responds promptly to client needs; responds to requests for service and assistance in a timely manner; communicates situations to administration. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. With no more than two (2) moving violations in the last three years; no more than one at fault accident in the last three years; no serious violation in the last three (3) years including: using false of fictitious registration, plates of driver's license; leaving the scene of an accident; driving on a suspended driver's license; murder or assault with a motor vehicle; theft of a motor vehicle or related incidents; driving under the influence (none in the past five years); reckless driving resulting in bodily injury or death (none in the past five years); negligent homicide. • Must be 21 years of age with a valid driver's license and five years of continuous driving experience Education/Experience: High school diploma or general education degree (GED). Language Ability: Ability to read and write English. Ability to speak effectively before groups of clients or employees of organization. Ability to write daily documentation of delivery of service, and complete incident reports correctly. Communicate daily information in a professional manner with clients, fellow staff, Path-Co administration, other service providers, and community members in verbal and written form. Communicate consumer information to fellow staff persons, lead staff, managers and/or directors as necessary. Follow and implement directives given by lead staff, managers or directors. Math Ability: Ability to accurately calculate figures and amounts such as daily data documentation, financial purchases, and receipt amounts. Reasoning Ability: Ability to solve problems. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Respect and implement Path-Co philosophies and professional business interactions. Know and follow Path-Co policies, procedures, and operation systems. Computer Skills: To perform this job successfully, an individual should have knowledge of Word Processing software and be able to complete typed reports as needed. Must have a smart phone and be able to access and utilize a variety of applications which are needed for this position. Certificates and Licenses: • Documentation of right to work in the United States of America. • Employee will earn passing scores and testing in the following, but not limited to: Colorado QMAP, CPR/First Aid, and Safety Care. Must be able to attend and show proficiency in scheduled and required trainings. Background Checks: All applicants are subject to the background investigations listed below. 1. Colorado Background 2. Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Must be able to wear a back pack with essential safety items. Must be able to provide supervision, crisis intervention, maintain professional boundaries and professional communication. Employee is frequently exposed to varying outdoor weather conditions: wet or humid outdoor conditions, extreme cold and extreme heat. The noise level in the work environment is usually moderate to loud. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. • Good physical stamina. • The employee must be able to lift and/or move up to 75 pounds. • Specific vision abilities required by this job include: depth perception and maintaining visual awareness of safety situations in the community and at base-site. • While performing the duties of this job, the employee is frequently required to: Stand; walk; run; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; talk, hear, write, and physically gesture; sit and stoop, kneel, crouch, or crawl, bend, squat, climb, twist, turn, reach above, and reach away from the body. • Must be able to physically assist people with transfers from a wheelchair to a changing bed, toilet, or vehicle. Must be able to push or pull a person in a wheelchair of moderate weight and safely manipulate wheelchair equipment, such as tie-downs and electric or manual lifts. Must be able to assist individuals with other medical equipment, such as Hoyer lifts, or anything deemed essential to the person's medical care. • Must be able to assist with ambulation and assist people with hygiene concerns, changing undergarments, toileting assistance, and peri-care.
