Student group looking for input from heart failure patients/caregivers

Hi -- I'm a member of a student team trying to build a solution for patients with congestive heart failure. We would really like to understand the patient experience as we learn more about the condition. We've created a short (10-15 minute) survey for those willing to help us out (see link below).
