Moderna Vaccination Clinic. ONE REGISTRATION PER PERSON. Moderna is a two-dose vaccination. The first dose will be administered on May 1, 2021. The second dose will be administered on May 29, 2021. Please ensure you can return on that date for the second dose, if not, do not register for the clinic. If you register and receive your first dose vaccine at this clinic on May 1, 2021, you will return at the same appointment time on May 29, 2021 for your second dose. Please read details below before registering: Pennsylvania residents 18 and older can receive the vaccine with proper proof of age, such as government-issued ID.You will have to fill out paperwork. Bring identification, and any medical insurance/prescription cards with you. If you are not insured, you may still receive the Moderna vaccines.There is a limit of two reservations per email for people who qualify.You can only make ONE appointment in your name. Duplicates will be deleted and reopened for reservations.You will receive an email from [email protected] (Foundation Secretary) which contains the paperwork that needs completed. You can print, complete the forms and bring to your scheduled appointment, OR you can complete the forms onsite (please arrive 10 minutes prior to your appointment).After vaccination, you will be required to remain for a 15 minute observation period.Please wear a mask covering your mouth and nose at all times at the vaccination site.
