Guest Home Care (Home Care Agency) needs a full-time Care Coordinator. Candidates should have a CNA or HCA certification and 5+ years of experience working as a caregiver, and at least 1 year working doing Care Coordination work (or equivalent). Care Coordinator: -will perform the scheduling of our client care -helping to staff new caregivers -resolving problems that arise -personally working the needed caregiving shifts when no caregivers are available -taking sales calls -performing prospect assessments and signing up new clients (commission paid) The Care Coordinator is a demanding position. REQUIRED: -Caregiving Experience: 5+ years -Care coordination (or equivalent): 1+ years -CNA (or NAC) or HCA Certification -ND (Nurse Delegation is not required, but must be obtained soon after hire) -Schedule Flexibility -Strong computer skills -Problem Solving skills -Good Judgement and Objective Decision Making -Mature, Team Player, Commitment to Excellence -Multi-tasking skills -Excellent communication skills -Pass a Finger Print Check & Background Check -Good driving record, valid driver's license, and current insurance RESPONSIBILITIES: - Manage the office - Client retention, home visits, and continuous communication - Help recruiting and onboarding of caregivers - Train and assist Caregivers, and do client-caregiver introductions - Personally Provide Caregiving Services (covering some shifts) to clients with a good attitude - Communicate responsibilities to caregivers and track their attendance - Maintain files - Keeping the Company Compliant - Work with Government Agencies when needed (ESD, LNI, etc.) - Performing client assessments and creating care plans - Will communicate with General Manager and Care Manager regularly -Taking new prospect calls & performing client assessments PAY/BENEFITS: - Great Pay (we will let you know the pay range when we talk)! - Commission Pay for New Clients - Paid Training. - Direct Deposit. - PTO (paid time off). - Paid Holidays. - Free Continuing Education. - After 1 year, $200 per month is paid toward your medical insurance. - Opportunity for Career Growth/Advancement. - We appreciate and support our staff!
