COVID-19 vaccination clinic for Chicago residents age16 and older in Phase 1 and Phase 2
Alderman Brian Hopkins, Alderman Walter Burnett, Alderman Brendan Reilly, and Alderman Michele Smith are co-hosting a Pfizer Vaccination Clinic in Chicago's historic Old Town neighborhood on Thursday, April 22, in partnership with Jewel-Osco Pharmacy.
This limited event is by appointment only and is on a first-come, first-served basis. On-site appointments or walk-in registration will NOT be available.
PLEASE READ OVER THE FOLLOWING EVENT DETAILS CAREFULLY:
This event is ONLY for Chicago residents over the age of 16 in Phase 1 and Phase 2.Participants will receive the first dose of the two-dose Pfizer Vaccine, and must be able to return on Thursday, May 13 for the second dose.Masks are mandatory. Temperature checks will be done prior to entry - if you are sick, please stay home.Upon completing your registration here, you will receive a confirmation email with a link to a “Vaccination Consent” form (a fillable .pdf file). You must complete/print this form and bring the copy with you to the event. Plan for your appointment to take one (1) hour from start to finish.
