Dr. Jason Hubbard and a team of nurses will be giving the Pfizer vaccine in the Community Center at Crestview Church of Christ on April 22. Come to the Crestview Church of Christ Community Center on Thursday April 22 at your designated time window. When entering from Delhi Rd, you will drive downhill around the west side of the facility and come around to see the community center. *If you are coming for your 2nd dose, please bring your vaccine card. *If you are under 18 you will need a parent or guardian's permission.
