From Purdue University Northwest: April 8, 2021 We at Purdue University Northwest are excited to share vaccination opportunities on the Westville and Hammond Campus. We are partnering with the Indiana State Department of Health and LaPorte county Health department and thus, we are excited to announce our campus vaccination days and times below. Getting vaccinated is perhaps the most important step we'll take as we work together to get back to normal. Please read this message carefully for details on how to schedule, what to bring to your appointment and where to go if you have questions. Who Is Eligible? Any PNW student, faculty and staff can go to either vaccination site. Hammond Our campus site will prioritize students in the first week in an effort to get them fully vaccinated before the end of the semester. All students (undergraduate, graduate and professional) age 16 and up, regardless of residency, can use the campus site at no cost. Those under age 18 will need to bring written parental consent. The second week we invite faculty and staff to sign up. However, we do not want to leave any appointments open so look for communication letting you know if week one has openings. This site is not open to the public. Westville The Westville site will be open to all PNW students, faculty and staff as well as the public. How to Schedule Hammond You will receive an email in the coming days from the Indiana Department of Health's scheduling partner Zotec ([email protected]) with a unique link to schedule an appointment at our campus site. The link in this email is unique to you and should not be shared. Please be patient in scheduling, understanding that not everyone will be able to receive a vaccination on the first day the campus site is open. Westville The phone number for PNW campus members to call for early registration is 1-888 580-1060 EXT 3401. Vaccination Sites: Hours and Location Hammond The clinic is located in the Nils Nelson Bioscience Building on the first floor in the east wing. The vaccination clinic will be open to following days and times: Dose #1 Pfizer April 13, 2021: 8 a.m. to 4 p.m. April 14, 2021: 10 a.m. to 6 p.m. April 15, 2021: 8 a.m. to 4 p.m. April 19, 2021: 8 a.m. to 4 p.m. April 20, 2021: 8 a.m. to 4 p.m. April 21, 2021: 8 a.m. to 4 p.m. Dose #2 Pfizer May 5, 2021: 8 a.m. to 4 p.m. May 6, 2021: 10 a.m. to 6 p.m. May 7, 2021: 8 a.m. to 4 p.m. May 11, 2021: 8 a.m. to 4 p.m. May 12, 2021: 8 a.m. to 4 p.m. May 13, 2021: 8 a.m. to 4 p.m. Westville The clinic is located in the DSAC Great Hall. The vaccination clinic will be open: April 15, 2021: 9 a.m. to 4 p.m. April 22, 2021: 9 a.m. to 4 p.m. This site is receiving Johnson and Johnson. Vaccination FAQs How Do I Complete Pre-Appointment Registration? After scheduling your appointment, you will have the opportunity to complete pre-appointment registration online, including supplying your insurance information, before your appointment. We strongly encourage you to do this to speed up the check-in process when you arrive at the vaccination site. What Do I Need to Bring to My Vaccination Appointment? You will need to show a photo ID (PUID card or other state-issued ID, such as a driver's license or passport) and your insurance information if you have medical insurance. You do not need to bring your physical insurance card, only your plan information. You will not be billed for the vaccination. How Do I Schedule My Second Dose? Hammond The state has allocated the Pfizer vaccine for our clinic, which requires two doses administered 21 to 23 days apart. We will schedule your second dose at the vaccine clinic immediately after receiving your first dose. Westville The LaPorte Health Department will have Johnson and Johnson, and no second dose is required. Other Scheduling Considerations As you're scheduling, remember to consider your individual circumstance and bear in mind factors such as: Are you planning to leave campus at the conclusion of the semester or are you staying for the summer? If you are leaving, will you be able to receive both doses before you depart or just the first, knowing there is a window of 21-23 days between doses? If you are not able to receive both doses before you leave, you will need to schedule your second dose of the Pfizer vaccination at your summer place of residence, so you should be sure the Pfizer vaccine is available where you will be. Consider your end of semester academic schedule and plan your vaccination around critical deadlines and exams as appointments allow. Take note of dates you want to avoid — again, knowing that your second dose needs to be administered 21-23 days after your first. Consider the potential impact of common and mild side effects on your classes, final projects and exams, particularly after the second dose, which can include fever or fatigue, generally subsiding within a day.
