Support Coach- Supportive Living/ Supported Employment Horizons creates and sustains person-centered opportunities for people with special needs where they live, learn, work, and play. The Support Coach- Supported Employment is responsible for teaching and modeling job-related skills for the individuals and implementing their professional, behavioral, and social programs. The Support Coach- Supported Living is responsible for ensuring the physical and emotional health and safety of each resident at the home where they reside. The Support Coaches help foster the independence of the individual, support them in achieving their goals, and facilitates opportunities for them to live a fulfilling and meaningful life. In this position you will: Model job skills for worker(s) and train worker(s) in tasks to be performed Balance needs/interests of each worker with employer needs/expectations Hold worker(s) accountable for job responsibilities and ensure job gets done satisfactorily each day Implement a program that includes goals and objectives as established by the person’s team in the Individuals Service Plan Administer medication as directed by the Manager in accordance with Physician’s Orders and DDS Medication Administration guidelines Be accepting of people's differences (e.g. abilities, styles, personalities, etc.) and adjust own approach to accommodate others Communicate in a way that is accurate, timely, positive, person-centered and respectful of the person’s privacy Complete all required documentation daily and participate in regular staff development sessions Keep manager(s) informed of incidents, behaviors, comments, etc., regarding resident(s) that are unusual, problematic or significant Intervene in emergency situations (e.g., self-injury, medical aggression) and according to an individual’s specific behavior management program utilizing training received from Horizons Benefits: Competitive hourly wage starting at $14.75 per hour Opportunity to move into a full-time position within a year of employment (dependent on performance) Part-time benefits include: 401K eligibility with a 3% employer match Vision, Pet, and Voluntary Life Insurance Full-time Benefits include: 401K eligibility with a 3% employer match Health, Dental, Vision, Pet, and Voluntary Life Insurance Short-term Disability and Life Insurance Requirements: The Support Coach is at least 21 years of age Must possess a high school diploma or equivalent Must possess a valid Connecticut driver’s license, acceptable driving record, and reliable transportation Must possess at least two years minimum experience working with people or an approved equivalent Must be able to pass a criminal background & license check performed by Horizons Must be able to pass a criminal background & license check performed by Horizons Physical Requirements - Ability to lift at least 50 pounds; ability to respond to critical incidents that may require protective holds and escorts; ability to assist individuals to enter/exit buildings, work sites, vans/vehicles, etc; ability to perform CPR and First Aid About Horizons: Horizons has been offering support to individuals with developmental disabilities and other special needs for nearly 40 years.
