COVID-19 vaccination clinic for Chicago senior citizens age 65 and older
On Saturday, April 10, 2nd Ward Alderman Brian Hopkins is hosting a COVID-19 Vaccination Clinic in Chicago's historic Old Town neighborhood for Chicago residents age 65 and over ONLY.
This limited event is by appointment only and is on a first-come, first-served basis. On-site appointments or walk-in registration will NOT be available.
PLEASE READ OVER THE FOLLOWING EVENT DETAILS CAREFULLY:
This event is for Chicago residents age 65 or older ONLY.Participants will receive the one-dose Johnson & Johnson (J&J) vaccine.After registering, you will receive a confirmation email with a pre-registration form attached to it. You MUST fill out the pre-registration form ahead of this event and bring the completed form with you.Bring your photo ID and insurance card with you. If possible, please bring a photocopy of these items with you to streamline your check-in process.At registration, you will be asked to present your Eventbrite ticket, received via your confirmation email. Please have it available on your cell phone OR printed out on a piece of paper.Wear clothing that allows vaccinators to easily access your upper arms. Clothing with short sleeves or no sleeves is best.Arrive no more than 10 minutes prior to your appointment slot.Temperature checks will be done prior to entry, and masks are required.Plan for your appointment to take one (1) hour from start to finish.Paid parking is available in a garage on site but is very limited.
