Easter outdoor worship service at Calvary United Methodist Church in Arlington, Virginia. I acknowledge: 1. I have not had 2 or more of the following symptoms of COVID-19 in the past 10 days: • Fever • Shortness of breath or difficulty breathing • Chills • Persistent cough • Flu-like symptoms • Diarrhea or intestinal upset • Fatigue • Sore throat • Headache • Muscle pain 2. I have not tested positive for COVID-19, or if I have, I have not exhibited any symptoms in the past 10 days. 3. To the best of my knowledge, I have not had prolonged exposure (unmasked within 6 feet for more than 15 minutes) to anyone who has tested positive for COVID-19. Those who are vaccinated are exempt from this question. 4. I will immediately notify my pastor if I test positive or exhibit 2 or more symptoms of COVID-19 (identified above) within 48 hours after attending In-Person Worship. I can answer ‘YES’ to all of the above questions. If you cannot answer ‘YES’ to all of the above, we ask you to wait before attending any in-person gatherings at church until you can answer affirmatively.
