We can now get our COVID-19 vaccinations in Moyock. This is open to everyone at least 18 years old. Please share. See below: Currituck Internal Medicine and Family Practice in Moyock, NC is giving first dose Moderna COVID-19 vaccinations. You will return for a second dose Moderna COVID-19 vaccination approximately 28 days later. You do not need to be their patient and you do not need to have medical insurance. You must be at least 18 years old. There is no out of pocket cost to insured or uninsured vaccine recipients. The quickest way to sign up is to send an email containing your: • First Name • Middle Name • Last Name • Date Of Birth • Email Address (They will assume your email address in the one from which your email originated if you do not provide an alternate email address in the body of your email.) • Phone Number (Please let them know if this is a mobile, home or work phone number. You may provide alternate phone numbers as well.) It is helpful but not required to send a scanned copy or picture of the front and back of your medical insurance cards and photo identification. If you prefer they can make copies of these for you when you arrive for your vaccine appointment. Making copies adds a little to your processing time. You may still get a vaccination if they do not have copies of your medical insurance cards and photo identification. The vaccine is provided without charge to Currituck Internal Medicine and Family Practice. However there are other expenses incurred by Currituck Internal Medicine and Family Practice in the process of administering, handling and storing the vaccine. Some medical insurances will provide reimbursement to Currituck Internal Medicine and Family Practice to help offset the expenses incurred for administration of the vaccine. Once your email is received your information will be entered in the North Carolina Department of Health and Human Services (NCDHHS) COVID-19 Vaccine Management System (CVMS). You will then receive an email from the CVMS asking you to complete the registration. The link for the NCDHHS CVMS recipient portal is https://covid-vaccine-portal.ncdhhs.gov Once you have completed the NCDHHS CVMS recipient portal registration Currituck Internal Medicine and Family Practice will contact you to schedule a vaccine appointment. If you are unable to send an email you may call to leave a voicemail with the above requested information. Please speak clearly and spell any uncommon, easy to confuse or otherwise difficult words. You do not need an email address to get a vaccine, however providing an email address expedites the process. Once your voicemail is received your information will be entered in the NCDHHS CVMS. If you provided your email address you will then receive an email from the CVMS asking you to complete the registration. If you did not provide an email Currituck Internal Medicine and Family Practice will contact you to complete the registration by phone.
