COVID Vaccination Clinic | Phase 1A Eligibility Only. Registration required. Hershey Pharmacy & Gift Shop in partnership with Shades of Greatness, Inc. and the Camp Curtain YMCA are providing a COVID Vaccination Clinic for Phase 1 Eligible Individuals [age 65+, and ages 18-64 w/high risk conditions]. Check www.health.pa.gov for high risk conditions. Pre-screening paperwork will be emailed to you. Please complete, print and bring to appointment. This will reduce wait time. Forms will be available on site, but may cause a delay in you receiving the vaccination in your preferred time slot if you are completing forms on site. Bring photo identification, along with any insurance and prescription cards. Appointment time and pre-screening paperwork will be provided in a separate email.
