concrete & paver walkway

I am building a 55 ft. cement walkway (with brick pavers on both sides) that lead to my exterior basement door. I have already dug the walkway and prepared the base. At this point I need someone to come and: 1) Provide and level concrete (55ft L x 20 in W x 2-3 in D) - you provide concrete and forms and etc 2) Set the brick pavers around the concrete - I have all the material (sand, pavers, paver guards, etc) I would like two quotes, the first just to do item #1, the second to do item #1&2. Ideally I would like to have this done as soon as possible (before Wednesday). Lastly, lets not waste both or our times with egregious prices, Im by no means a cheap person, and am willing to pay fair prices.
