Gellert Health Services now offers COVID-19 vaccine appointments for eligible individuals who live or work in San Mateo County. In an effort to provide convenient access to vaccination, Gellert Health Services is offering appointments for eligible individuals who live/work in San Mateo County. The vaccine we are currently offering is Moderna. ELIGIBILITY Based on local guidelines, you are eligible for the vaccine if you live/work in San Mateo County AND are one of the following: Age 65 or olderAge 16-64 with serious health conditionsPhase 1A/1B Frontline Workers: Healthcare workers, fire fighters, police officers, corrections officers, food and agricultural workers, United States Postal Service workers, manufacturing workers, grocery store workers, public transit workers, and those who work in the educational sector (teachers, support staff, and daycare workers.)REQUIRED DOCUMENTS Please be sure to bring the following with you: Valid Photo IDProof of addressProof indicating eligibility (recent paystub, work badge, etc)Insurance cards, Medicare card, Medi-cal card (if applicable)Once registered, you will receive a confirmation email--please print it out or show it on your mobile device.
