(AP) — An overwhelmed 911 system, chaos and delays hampered the law enforcement response to the November 2018 mass shooting at the Borderline Bar & Grill in Thousands Oaks that left 12 innocent people dead and 23 injured, a report released Wednesday concluded. The 88-page after-action report cited a series of problems that created confusion after a Marine veteran opened fire with a handgun inside the Borderline Bar & Grill on Nov. 7, 2018.
