Description

Sign up for a time to come into the front office to register your upcoming Kindergartner for the 2021-2022 school year. Please have your packet already filled out with the following documentation ready to be copied (we are happy to make a copy for you). Bring with you the Original Birth Certificate, Current Immunizations, proof of residency (mortgage statement, city bill, or rental agreement are examples of proof of residency), and any court documentation applicable to the Kindergartner (adoption paperwork, divorce decree, or name change documents are examples). If you haven't picked up a kindergarten packet please come into the front office of Silver Lake and pick one up.
