Registration and consent form completed prior to the event. You will be contacted with a specific date and time of the vaccination. We will be able to vaccinate the following populations: • Persons 60 years of age and older • Health care personnel with direct patient contact; (Will need employment badge with picture and title or medical license) • K-I 2 school employees 50 years of age and older; (Will need employment badge or proof) • Sworn law enforcement officers 50 years of age and older; and (Will need employment badge or proof) • Firefighters 50 years of age and older. (Will need employee badge or proof)
