Grab this opportunity to show up for seniors while enjoying a great time with other area profesionals! Do NOT miss this event! Registration is open at 7 AM with an 8 AM shotgun start. The tournament activities will include on course game with prizes, silent auction, 50/50, and raffles. REGISTRATION IS $450 PER FOURSOME AND HOLE SPONSOR. DBA ElderPoint Services has been serving Polk County since 2005. We are a 501(c)3 non-profit with a simple mission: to improve the quality of life for senior residents of Polk County at their point of need, through services that empower healthy, independent living. Our current programs are our non-emergency medical transportation service and Caregiver Resource Center. We address needs as they arise for seniors and their families/caregivers, supporting the navigation of the senior care network for options tailored to the needs and desires relevant to the individual senior. During 2020 we provided 8,017 FREE rides to seniors for life-sustaining medical-related appointments. This was up by 250% compared to 2019.
