BON VOYAGE TO THE CLASS OF 2021! You are invited to the 64th Annual Senior Style show. We hope you will join your Senior for this time-honored St. Agnes Academy tradition!Thank you for registering, there is NO COST to attend! Due to COVID-19 safety protocols, registration is limited to two guests per senior. Guests will be seated in pods of two (2) and you will have two bottled waters at your POD. Check-in times and seating assignments will be assigned prior to the event in order of registration. Please be sure to include your senior’s name on the form to confirm your reservation. The registering guest (Guest 1) will be contacted the week after registration closes on Friday, April 12 with their POD number, a map showing where their POD of two seats is located, and an arrival time. If both guests are not arriving together, please make sure Guest 2 knows your assigned POD number. Upon arrival, please remain socially distanced and proceed directly to your POD. There will be volunteers to assist you. Check-in between 3 - 4 p.m.* Program & Style Show: 4 p.m. Please refer to the website for up-to-date information at www.st-agnes.org/seniorstyleshow Registration closes at 5 p.m. Friday, April 2 This event will follow proper COVID-19 safety guidelines. By attending the 2021 Senior Style Show you hereby agree to the following: I understand that registering for a POD and thereby attending the 2021 Senior Style Show as a guest is strictly voluntary. I understand at-risk populations are strongly encouraged to stay home. I have not displayed any of the following symptoms during the fourteen-day period preceding the performance date: cough, shortness of breath or difficulty breathing, chills, repeated shaking with chills, muscle pain, headache, sore throat, loss of taste or smell, diarrhea, or feeling feverish or a measured temperature greater than or equal to 100.0 degrees Fahrenheit. Moreover, I have not had close contact with a person who is lab-confirmed to have COVID-19 within the last fourteen days.I understand that the following protocols will be in place for the event. I agree to abide by these protocols: maintain social distancing, wear a mask that covers the mouth and nose, remain in my seat unless moving is absolutely necessary, avoid walking around and socializing with others, practice proper hand hygiene and cough etiquette, use hand sanitizer upon entry and exit of Bayou City Event Center.I understand that Coronavirus Disease 2019 (COVID-19), a severe respiratory illness, has spread globally, including the United States and that SARS-CoV-2, the virus that causes COVID-19, is extremely contagious. The World Health Organization has declared COVID-19 to be a worldwide pandemic, and federal, state, and local governments, as well as federal and state health agencies, have instituted procedures and guidelines in an attempt to limit the spread of COVID-19. I recognize that there is a still a risk of contracting the virus, particularly in group settings, even if I follow precautions designed to lower the risk of infection. I voluntarily accept any and all responsibility for myself, my Student’s and/or Minor Children’s safety and welfare while participating in or attending this performance, with the full understanding of the risks involved. I hold harmless and release ST. AGNES ACADEMY, THEIR TRUSTEES, SCHOOL BOARDS, REPRESENTATIVES, MEMBERS, OFFICERS, EMPLOYEES, VOLUNTEERS, CONTRACTORS AND/OR AGENTS of any and all responsibility and liability for any injury or claim resulting from my or my Student’s participation in (if applicable) or Minor Child’s attendance at (if applicable) this event. In consideration for being allowed to participate in and/or attend the performance, I, for myself, my heirs executors and administrators, my Student and Minor Children and their heirs, executors and administrators release and forever discharge ST. AGNES ACADEMY, THEIR TRUSTEES, SCHOOL BOARDS, REPRESENTATIVES, MEMBERS, OFFICERS, EMPLOYEES, VOLUNTEERS, CONTRACTORS AND/OR AGENTS of all liability, claims, actions, damages, and/or costs/ expenses my Student, my Minor Children, or I may have against them, arising out of or in any way connected with my, my Student’s, and my Minor Children’s participation in this event. I understand that this release/waiver of liability applies to ANY claim, even those based upon the negligence, actions or inactions of those referenced above. READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO PARTICIPATE IN AND ALLOW YOUR MINOR CHILDREN TO ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT EVEN IF THE SCHOOL USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOU, YOUR STUDENT, OR YOUR MINOR CHILDREN MAY BE INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM, YOU ARE GIVING UP YOU AND YOUR MINOR CHILDREN’S RIGHT TO RECOVER ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR STUDENT OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY.
